Agreement to the documents in the previous is a requirement of certification. You may print and sign the PDF or complete the Google Form. Printed and signed documents must be emailed to your trainer and office@trainingdoulas.com

The Google form does NOT require you to use a Google email. If you submit the form, please be sure to submit the assignment with a note that you completed the form. If you use an email address for the form that’s different from the email we have on file for you, please write that email address in the assignment so we can locate your form.
